How do I Apply for UP State Health Card Online?

To get benefited through Cashless Treatment Scheme Uttar Pradesh you can follow steps below to get Health Card:

• Open UP state Govt employees and pensioners have to visit upsects.in or uphealth.up.nic.in.


• Move your cursor to “Employee/ Pensioner Gateway”. A drop down will open.


• Click on “Apply for State Health Card” in the drop down menu.


• On the next page, a pop-up will appear that contain important information listed below

1. In the Employee / Pensioner Registration Form, in the point number 9, the actual salary will be marked on the basis of the Sixth Pay Commission.


2. Those pensioners who have retired from the pay band of the 5th Pay Commission, seek the salary matrix of the Sixth Pay Commission available in "GOs" to mark the equivalent pay band of their Sixth Pay Commission.


3. Those state employees who are receiving the salary of the Seventh Pay Commission, with the help of the payroll matriculation of the seventh pay commission, pay the equivalent salary of the Sixth Pay Commission.
